Accelerated theta-burst stimulation using individualized fMRI-based targets in adolescents with depression

Background: Accelerated intermittent theta-burst stimulation (aiTBS) using resting-state functional magnetic resonance imaging (fMRI)-based neuronavigation was recently cleared by the FDA for use in adults with depression. The objective of this study to gauge clinical utility fMRI-guided adolescents Methods: Three individuals (male, 16-17 years) underwent fMRI scanning prior treatment. Individualized left dorsolateral prefrontal cortex (DLPFC) brain targets were derived each patient based on connectivity subgenual anterior cingulate cortex. aiTBS treatments administered via MagVenture MagPro R30 system and a Cool-B65 coil, paired Localite TMS Navigator. Patients received 50 sessions over five days (8 11 per day, spaced minimum minutes). Self-reported depression symptoms evaluated pre- one-week post-treatment Patient Health Questionnaire-9 (PHQ-9). Results: Baseline PHQ-9 scores 20, 19, 19 indicating moderately-severe severe symptoms. Following treatment, decreased 3, 2 12, respectively two patients achieved remission no longer had depressive (85% 89% reduction symptom severity). One considered non-responder, however still 37% severity. Conclusions: treatment well tolerated all three patients.
